Overview
Purpose Provide information, assistance and support to senior citizens and adults with physical disabilities participating in the consumer direction within CHOICES, (the self-directed service option within Tennessee's Home and Community Based Medicaid Service Waiver) to assist them with the responsibilities of self-direction, including the responsibilities of being an employer.
Reporting Relationships
The Support Broker will report to the Senior Program Administrator - Resource Consultants.
This is a remote position in the Memphis area as well as Middle TN especially the Upper Cumberland area (Cookeville, Jamestown, Crossville).
Responsibilities
Marketing and Outreach:
Assist state agency representatives in marketing CHOICES to prospective Participants, family members, representatives, service providers, others through development, revision and distribution of program materials, and through individual and group presentations, home visits and telephone contacts.
Explain the roles and responsibilities of self-direction in CHOICES to members and their appointed representatives, including the responsibilities of being an employer, hiring and supervising workers.
Explain the roles and responsibilities of PPL in providing Financial Management services to members/representatives, including the responsibility for payroll processing and payment, and the responsibility for tax withholding and reporting.
Explain the roles and responsibilities of PPL in providing Support Broker services to members/representatives, including the responsibility of assisting with the enrollment process and interfacing with the member's care coordinator to meet the needs of the member related to Consumer Direction.
Participant Enrollment, Orientation and Training:
Assist interested and eligible participants to enroll in CHOICES consumer direction, including completion and submission of enrollment packets, employer packets and employee packets.
Enter member/representative information into PPL portal used for project management and reporting and forward enrollment packets for member/representative and workers to PPL financial operations center for processing.
Provide members/representatives with practical skills training as needed in areas such as: developing a job description for workers, recruiting, selecting, hiring, training and supervising employees; and communication with support brokers and care coordinators.
Communicate with member's care coordinator upon initial receipt of referral and throughout the enrollment process so as to keep them informed of any issues and/or potential enrollment delays.
Assist Participants to Recruit and Retain Employees:
Assist member/representatives as needed to identify, recruit, select, hire, train, schedule, manage/supervise, and terminate employees. Collaborate with PPL staff to ensure that all employees have completed required background and registry checks prior to provision/payment for services.
Assist members/representatives as needed to provide mandatory training required of all employees/employers prior to providing paid services. Collaborate with PPL staff to verify that all employees/employers have completed required training prior to the provision/payment of services.
Assist members/representatives as needed to provide ongoing annual training/certification required of all employees. Collaborate with PPL staff to verify that all employees have completed required ongoing annual trainings.
Assist PPL management and staff to identify workers interested in being included on the internal registry/directory of potential workers.
Monitoring and Reporting:
Monitor member health, safety and welfare through enrollment and initial planning process.
Monitor worker adherence to member schedule and prescribed clock in to the Electronic Verification System (EVV)
Report member's inability to follow schedule as written in the POC and worker's failure to clock into the EVV system according to the prescribed schedule to the Care Coordinator.
Assist care coordinator to reeducate member and workers in prescribed schedule, proper clock in procedures and the impact on payroll/member services if POC is not followed and worker fails to clock into the EVV system correctly.
Maintain member and employee files and records according to prescribed HIPAA standards.
Maintain member/worker communication by creating and updating support tickets in the PPL web portal.
Document and report evidence and observation of member or representative inability to self direct.
Report and respond to all member/representative complaints regarding PPL services using required reporting processes and systems.
Report and respond to all member/representative grievances using required reporting processes and systems.
Report and respond to all critical incidents, including any and all allegations or reports of suspected abuse, neglect, fraud and exploitation.
Report and respond to all incidents involving members/workers to all required agencies.
Qualifications
Minimum
Qualifications:
Bachelor's Degree in human services field from an accredited institution AND at least one year professional experience working with persons who are elderly or disabled
Or
Bachelor's Degree in any field from an accredited institution AND at least two years professional experience working with persons who are elderly or disabled.
Or
Associate's degree in human services field AND at least 5 years professional experience working with persons who are elderly or disabled. Duties and Responsibilities

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
